Our small house model means our elders receive personalized attention for skilled nursing, rehabilitation, memory care, and long-term care.

Perrysburg SeniorLife Neighborhood is more than a nursing home — it’s a new and innovative style of nursing care that encourages personal choices and provides an individualized approach to caregiving.

There are many ways that our small house amenities set us apart from traditional nursing homes:

  • Maximum of 12 elders/rehab guests in each small house
  • Private suites and baths near shared living areas
  • Elder-directed menus and activity planning
  • Licensed nurses and nurse assistants on duty 24 hours per day
  • Fenced, shaded outdoor courtyard
  • Central hearth with adjacent open kitchen and dining area
  • One large dining table for home-cooked, family-style meals
  • Low staff turnover rate with dedicated caregivers for each house
  • Therapy services in addition to memory support and long-term care
  • In-room personal medicine cabinet

Otterbein Perrysburg Has Grown!

We’ve added 10 new private suites to our neighborhood — paving the way to make our small house philosophy of care more accessible to everyone. Our new suites feature remote-controlled personal heating, complimentary Wi-Fi, and spacious bathrooms with walk-in showers.

About Perrysburg

Located in Wood County, the city of Perrysburg is a great mix of historic charm and modern convenience along the banks of the Maumee River. The historic downtown area features unique shops and restaurants and beautiful Victorian architecture, and the Town Center & Levis Commons houses a modern shopping center with national retail stores and restaurants.

With close proximity to Toledo, Perrysburg provides direct access to a wide variety of venues for shopping, dining, entertainment, and more. Perrysburg is also the location of the Fort Meigs State Memorial, a War of 1812 battlefield site.

Perrysburg SeniorLife Neighborhood is a group of five homes on cul-de-sacs in an established residential area in Perrysburg, Ohio. Each home features 12 private rooms with private baths, a communal kitchen and living room, and a secure backyard patio. This isn’t just a place where our elders stay — it’s their home.

How do we do it? We give our elders the power of choice. When to get up, what and when to eat, how to spend the day — these are all choices our elders are free to make on their own, just like at home. There are no rigid schedules or strict mealtimes; instead, independence is key.

Elders at Otterbein Perrysburg have access to 24-hour care from a small team of licensed nurses, nursing assistants, and therapists. There’s also the emergency call system. Elders may be living here on a permanent basis, or temporarily for rehabilitation after an injury or illness. Either way, they’re treated as part of the Otterbein family.

Do all residents live here permanently?

Some elders, also known as residents, are considered long-term care elders who have made Otterbein Perrysburg their permanent home. Other elders may stay here temporarily for rehabilitation after an injury or illness. Either way, they’re treated as part of the Otterbein family.

Is there a nurses’ station in each small house?

There are no nurses’ stations in the small houses at Otterbein Perrysburg. Instead, elders have secure, personal medication and supply cabinets in their private suites.

What is the staff turnover rate there?

There is a low staff turnover rate in our small house neighborhoods, including Otterbein Perrysburg. A small, dedicated team of interdisciplinary caregivers is assigned to each small house. This encourages meaningful connections and a team that knows individual elders’ likes and dislikes.
