About

Transition from Hospital to Home with Post-Acute Care

Enhance your recovery and goals for returning home after a surgery, illness, or unexpected injury with professional post-acute care services.

At Otterbein, our post-acute care team focuses on providing a seamless transition from the hospital to a short-term stay at one of our 15 campuses to home. While short-term care is usually less than 30 days, those few weeks of skilled nursing care can be crucial to your healing.

Short-term Care at Otterbein SeniorLife

You can receive professional, short-term post-acute care at our six SeniorLife Communities and at our nine SeniorLife Neighborhoods with locations throughout Ohio and in Indiana.

Your Otterbein transition team of skilled nurses and therapists works in conjunction with your doctors, hospital discharge department, and/or home health agency to get you back on your feet as soon as possible. As you progress and recover, you’ll learn how to manage your own needs, such as recognizing when to contact your health provider. And while you’re here, you can enjoy all the amenities Otterbein has to offer.
